Complete a New Service Request form online and e-mail the form along with the required documents to WaterServiceRequest@mlbfl.org and receive a response within two working days:

Fax a completed New Service Request form along with the required documents to: (321) 608-7199

OR

Contact the Utility Billing Department at (321) 608-7100

OR

Bring completed form along with the required documents to the Utility Counter located on the first floor at City Hall, 900 E. Strawbridge Ave., Monday – Friday, from 8:30 a.m. to 5 p.m.

To establish a new utility account, please provide the following:
Customer's Full Name
Service Address
Driver's License or ID
Documentation of Lease (rental) or Ownership (proof of purchase)

Deposit:
The deposit amount varies and may be billed on the first bill for customers without outstanding balances. A $20.00 service initiation fee will also appear on the first bill.  Past due bills are subject to disconnection.
